FCCPT will re-issue a lost or damaged Type 1 Certificate. The damaged certificate must be mailed to our offices. Once received, a new certificate will be issued to you.
In the event that the certificate is lost, you must submit a notarized affidavit of loss. Once we receive the affidavit of loss, we will add a service for your certificate replacement and notify you via email when you can log in and make the payment. Once the payment is processed, we will reissue your Type 1 Certificate.
