If your Type 1 Certificate (Health Care Worker's Certificate) is lost or damaged, FCCPT can issue a replacement.
There is a fee for the reissued certificate.
If Your Certificate Is Damaged
Mail the damaged certificate to FCCPT. Once the certificate and payment are received, a replacement certificate will be issued.
If Your Certificate Is Lost
Submit a notarized Affidavit of Loss to FCCPT by Uploading it on the FCCPT website or by mail. After we receive the affidavit, a replacement certificate service will be added to your file and you will be notified by email when you can make payment.
Once payment is received, FCCPT will issue a replacement Type 1 Certificate. Replacement certificates are typically processed within approximately 3 weeks after payment.
Note
Replacement certificates are issued only after the required documentation is received and payment has been processed.
If you have questions, submit a ticket at FCCPT Help Center.
