Complete the FCCPT Higher Education Revision Form to make changes to your education information. Download and complete the form, then upload it through the FCCPT website at www.fccpt.org.
PT Licensure Information
Complete the FCCPT PT License Revision Form to make changes to your physical therapy licensure information. Download and complete the form, then upload it through the FCCPT website at www.fccpt.org.
Important: Submitting the appropriate revision form helps ensure that your updated education or licensure information is added to your FCCPT application.
