Information You Will Need
FSBPT ID
If you have an FSBPT ID number, be prepared to provide it on the application.
Exam History
If you have taken and passed the National Physical Therapy Examination (NPTE), be prepared to provide the date you passed the exam.
License or Registration History
You are required to report any licenses or registration you have held. Have the following information available:
- Name of licensure/registration authority
- Date of initial licensure/registration
- Expiration date (if applicable)
Secondary Education History
Have the following information available for your secondary education:
- Name of the institution
- Dates of attendance
- Name of degree earned
- Date awarded or conferred
Physical Therapy Education History
For each physical therapy program attended, provide:
- Name of the institution
- Dates of attendance
- Name of degree earned (if applicable)
- Graduation or completion date (if available)
Non-Physical Therapy Higher Education History
For any additional college, university, or higher education programs that are not physical therapy, provide:
- Name of the institution
- Dates of attendance
- Name of degree earned (if applicable)
- Graduation or completion date (if available)
