If your employer has requested verification of your physical therapy education, you may need to request an official report from FCCPT or have a verification form completed. The steps you need to take will depend on whether your employer requires a formal FCCPT report, a completed verification form, or both.
What does “verification of education” mean?
Employers may be asking for one or both of the following:
- An official FCCPT report confirming that your education has been reviewed and whether it is equivalent to U.S. standards

Completion of an employer verification form, which may ask FCCPT to confirm details such as:
- Dates of attendance
- Degree awarded
- Whether a Healthcare Worker’s Certificate was issued
Both types of verification must be completed through FCCPT’s official processes and require your authorization.
What you need to do
1. If your employer has a verification form
Some employers require a form to be completed that verifies details such as:
- Dates of attendance
- Degree awarded
- Whether FCCPT issued a Healthcare Worker’s Certificate
In this case:
- You must complete and submit FCCPT's Release of Information form to FCCPT before your employer contacts us. (We do not accept other organization's authorization forms.)
- FCCPT cannot complete or return verification forms without your authorization
2. Determine if you already have an FCCPT report
- If you previously applied for a service (such as an Educational Credentials Review), you can apply for a Duplicate Report
3. If you do not have a report yet, apply for a service
You will need to apply for an FCCPT evaluation service, such as:
- Educational Credentials Review (ECR) – confirms whether your education is substantially equivalent to a U.S.-accredited PT program
This type of report is commonly used for:
- Employment verification
- Licensure
- Other official purposes requiring proof of education
4. Request that your report be sent to your employer
Once your application is submitted (or if you already have a service in progress):
- Log in to your FCCPT account
- Add your employer as a report recipient by applying for a Duplicate Report
5. Confirm your employer’s requirements
Employers may have specific instructions, such as:
- Requiring the report to be sent directly from FCCPT
- Requesting a specific service or type of report
- Providing their own verification form
Confirm these details with your employer before submitting your request to avoid delays.
