If you have received your completed evaluation and feel that your school didn’t submit all the necessary information before the review started, or if the initial details were incomplete or inaccurate, you can ask your school to provide additional documents to FCCPT and apply for a Re-evaluation.
Supplemental Documents
Additional documents might include the following:
- a more detailed syllabus
- proof of missing courses
- clinical education certificates showing internship hours
- licensure documents
- class materials such as lecture notes and PowerPoint presentations
Submission of Documents
Accepted documentation includes:
- documents sent directly from the school
- documents that were in use during your attendance
- class materials (like PowerPoint presentations)
Documentation that is not accepted includes:
- documents sent by the applicant
- letters from schools stating that content was taught
- documents not in use during your attendance
- class materials (such as PowerPoint presentations) that don’t clearly indicate which class they are from
