FCCPT requires academic documents to be sent directly from the educational institution. If your school has closed and cannot send the required documents, follow the steps below.
1. Contact the Appropriate Government Agency
Many countries have government agencies that maintain academic records for schools that have closed.
Locate the appropriate government agency in the country where you received your education and contact the agency to determine whether it can provide your official academic documents directly to FCCPT.
2. Contact the State Board of Physical Therapy
If the government agency is unable to provide your academic documents directly to FCCPT, contact the state board of physical therapy in the jurisdiction where you are seeking licensure.
FCCPT must receive approval in writing from that state board before we can use academic documents that were not provided directly by your school.
Important
Do not submit alternative academic documents to FCCPT without first obtaining the required approval from the state board of physical therapy.
