Some FCCPT forms are password protected to prevent changes to the form’s formatting or required content. You do not need a password to complete the form.
Try the following:
- Download the form to your computer.
- Open the downloaded file in Adobe Acrobat Reader rather than in your web browser.
- Enter your information in the available fields.
- Save the completed form to your computer.
If the fields cannot be completed after following these steps, you can print the form out and complete it by hand.
